Our interdisciplinary care team provides skilled nursing services and physician-ordered rehabilitation based on each resident’s assessed needs. Care plans are developed collaboratively and adjusted as appropriate to support medical stability, functional goals, and daily care needs.
Our facility includes a 33-bed sub-acute unit and a secured dementia unit. Services may include skilled nursing care, respiratory support, medication management, wound care, and rehabilitation therapies as ordered by a physician.

Our post-acute care unit provides skilled nursing services and physician-ordered rehabilitation for individuals transitioning from a hospital stay. Care focuses on medical management, monitoring, and therapy services based on assessed needs and care plans.
Licensed nursing staff are available 24 hours a day, with rehabilitation services provided as ordered by a physician to support recovery goals.
Our long-term care unit supports individuals who require ongoing nursing care and assistance with daily activities. Services are provided according to individualized care plans developed through assessment and interdisciplinary collaboration.
Residents have access to nursing support, structured activities, and opportunities for family participation in care planning, in accordance with facility policies and resident preferences.
